신경 줄기 내부에 낭종(물혹)이 생겨 신경계의 마비를 일으키는 희귀질환인 ‘신경내 결절종’의 복잡한 발생 기전을 밝혀낸 연구 결과가 나왔다. 치료가 어렵던 이 질환에 더욱 정교한 수술 치료가 가능해질 것으로 전망된다.
가톨릭대 서울성모병원 신경외과 손병철 교수와 미국 메이요 클리닉 신경외과 로버트 스피너 교수 공동 연구팀은 15일 이 같은 내용의 연구 결과를 발표했다. 연구진은 2016년 해당 질환에 관한 사례 보고 이후 지속적으로 인체 여러 부위에서 나타나는 발생 사례를 추적·수집하며 발병 과정을 분석해왔다.
신경내 결절종은 관절 내부를 채우고 있는 액체인 활액이 여러 갈래로 갈라지는 신경계를 타고 따라 역류해가면서 신경 줄기 안에 물혹을 형성해 말초신경 마비를 유발하는 질환이다. 체계적 연구가 진행되기 전에는 원인이 불분명하고 수술 후 재발도 잦아 영구적인 신경 손상을 남기는 난치성 질환으로 분류됐다.
메이요 클리닉 연구진은 2016년 결절종 중에서도 이전까지 알려지지 않았던 ‘신경곁조직 아래막’ 결절종을 최초 확인했고, 2018년에는 손 교수가 해당 유형의 결절종이 종아리를 지나는 비골신경의 마비를 유발하는 사례와 그 치료 과정을 학계에 알렸다. 이어진 연구를 통해 이 결절종은 신경섬유에 더 밀착해 있고 신경 줄기를 따라 매우 광범위하고 빠르게 전이될 수 있다는 사실이 확인됐다. 연구진은 이런 특성 때문에 일반적인 낭종 제거술로는 완치가 어려우며 관절과 연결된 신경 갈래를 근본적으로 차단하는 정교한 수술 기법을 적용해야 한다는 점도 함께 제시했다.
연구진은 이후 종아리 외에도 팔꿈치와 허리, 엉덩이 등 인체 전반의 말초신경 곳곳에서 이 질환이 발생하는 사례를 찾아 임상 데이터를 축적해왔다. 그 결과, 신경곁조직 아래막 결절종은 신경 줄기뿐 아니라 여러 갈래 가지처럼 뻗어가는 신경에도 확산되는 것으로 나타났다. 이런 확산 과정이 활액이 이동하는 미세한 통로를 따라 진행되므로 재발 없는 치료를 위해선 낭종이 이동하는 복잡한 해부학적 구조와 경로를 명확히 파악해야 한다는 치료 방침이 나왔다.
병의 진행 기전이 밝혀진 덕에 연구진은 더욱 정교하고 근본적인 치료가 가능해져 그동안 원인 모를 재발과 마비로 고통받던 환자들에게도 치료 효과가 나타날 것으로 전망했다. 손병철 교수는 “이번 연구 성과는 단순한 질환 보고를 넘어 신경내 결절종 환자들이 정확한 해부학적 진단을 바탕으로 마비의 위험 없이 완치될 수 있는 임상적 근거를 마련했다는 점에서 큰
